Transition Planning means deciding how best to use the high school years in order to ensure that the teen emerges at graduation as well prepared as possible to lead a fulfilling, independent life. Transition goals and services should be written into the IEP (Individualized Education Plan) and implemented when the student is 15 years old, so discussions should begin at age 14. Transition plans are supposed to be based on the vision statement of the teen student, but this is not so easy to articulate. This workshop is designed to get teens thinking about the lives they want to lead as adults, how to convey their vision to teachers and parents, and how to identify and acquire the skills they will need to reach their goals.
